Right Size Your Tools
There is no such thing as a perfect process. The basis of continuous improvement hinges on the pursuit of perfection and when on feels there are no problems, one is simply not looking hard enough.
Seeing the problem is often hard to do, especially in a culture where workers fear their jobs are in danger if problems are discovered. Often, they simply hide them. You also have to with legacy problems where extensive workarounds have become just part of the everyday work.
How do you see the problems? Create a company of problem solvers. Map out all of your processes with as a team, talk about their challenges, let them offer possible counter measures, and most importantly, get out of your office and go to where the work is done and interact.
